Financial Assistance Financial Assistance is available though the Community Options
Program (COP).
What is the Community Options Program? The Community Options Program (some people abbreviate it to COP
Program) is a system of assessing need and bringing together
services for people who cannot live independently because of
long-term medical, physical or mental problems. Some of these
elderly and disabled people prefer not to go to a nursing home
or institution when they require care. For the people who choose
to live at home, the COP program tries t0 bring together services
to assist them in living at home. The services vary with the
individual needs of the client. Some examples are chore service,
housekeeping, nursing care and therapy.
Who is eligible for the COP? The Community Options Program is for Vilas County residents of
all ages who are: frail elderly, or physically disabled, or developmentally
disabled, or chronically mentally ill, or chemically dependent
and
have a long-term or irreversible illness or disability;
and
require ongoing assistance and periodic medical
or restorative care, at a level reimbursable in a nursing home;
and
without COP would require residential and medical
services in a nursing home or State Center for the developmentally
disabled,
or who are current residents of such facilities; and
are included
in the county plan for gradual implementation,
are chronically
mentally ill and at risk of repeated hospitalization,
have
Alzheimer’s diagnosis.
How much does it cost? Assessments and case plans are free to the persons who meet the
COP eligibility criteria. Persons who would be eligible for Medical
Assistance if they entered a nursing home are also eligible for
COP service funds. COP service funds can be used to purchase
any services or equipment, which would enable the disabled person
to continue to live in a non-institutional setting. For example,
COP funds might be used to pay a homemaker of personal care aide,
or to purchase a hospital bed or build a wheelchair ramp. Individuals
who do not qualify for free Services may be eligible to purchase
services at a reduced cost based on their ability to pay.
How does the program work? For any applicant found eligible a meeting is set up for an assessment.
A social worker will talk to you, your family and your medical
providers. With your help, they will figure out what help and
services you need to assist you to live at home. They will also
make a plan of how to get those services for you. You, the individual
applicant, will always have the final say. Consideration of cost,
service, availability, and family support will all be part of
the process. You have the right to refuse an assessment regardless
of your pay status. You have the right to stop the application
process at any time or to discontinue services at any time. Even
if you apply to our program, you will not be denied admission
to a nursing home on that basis.
